Colecistectomía laparoscópica por colecistitis crónica litiasica en paciente con situs inversus totalis, reporte de caso y revisión de la literatura

Abstract
Introducción: el situs inversus totalis es una entidad autosomica recesiva poco frecuente con una incidencia de 1:4.000 a 1:20.000. La colecistectomía laparoscópica es el estándar de oro en el tratamiento de la colecistitis crónica litiásica y puede realizarse de manera segura en pacientes con situs inversus totalis. Objetivo: presentar el caso clínico de una paciente con situs inversus totalis y colecistitis crónica litiásica tratada mediante colecistectomía laparoscópica. Material y métodos: se reporta el caso clínico de una paciente de 24 años con situs inversus totalis y colecistitis crónica litiásica, tratada mediante colecistectomía laparoscópica en el Hospital General Delfina Torres de Concha, Esmeraldas. Se utilizó un equipo de laparoscopia básico, con posición americana invertida y cuatro puertos de trabajo. Resultados: Se realizó una colelap en un paciente por primera vez con colecistitis crónica litiásica y situs inversus, de manera exitosa y sin complicaciones. Conclusiones: El situs inversus totalis es una alteración estructural poco común en nuestra región. El tratamiento de la colecistitis crónica litiásica mediante abordaje laparoscópico en estos pacientes es factible, seguro y reproducible en un hospital de segundo nivel del ministerio de salud publica.
